5.30.010 Filing of financial statement required.
Any organization qualifying as a “bona fide” charitable or nonprofit organization under Chapter 280, Laws of 1971, First Extraordinary Session, as amended, intending to conduct in the City of Mountlake Terrace those bingo, raffle or amusement games which under said Chapter 280 are not subject to state criminal law penalties, shall no less than 10 days prior to the commencement of any such activity, file with the City Treasurer of the City of Mountlake Terrace a sworn financial statement in such detail as may be required by said Treasurer, covering the operation of any such activity conducted during said preceding quarterly period, together with another such sworn declaration of intent if such activity is to be continued. (Ord. 860 § 1, 1972).
5.30.020 Information required on statement – Amendments.
The declaration of intent and financial statement required in MTMC 5.30.010 shall be in such form as shall be prescribed by the City Treasurer of the City of Mountlake Terrace and the declaration of intent shall require disclosure of the name and address of the organization and the time and place said organization intends to conduct such activity and such declaration of intent and financial statement shall require disclosure of such other information as may be required by the said Treasurer to show compliance with Chapter 280, Laws of 1971, First Extraordinary Session, as amended. It shall be the duty of the organization to file with the said Treasurer, from time to time, such amended declarations as may be necessary to provide the public with a current and accurate statement of the information contained in such declaration which disclosed compliance with said Chapter 280. 5.30.030 Filing fee.
The Treasurer of the City of Mountlake Terrace shall file as a public record, declarations of intent and/or financial statements provided for in MTMC 5.30.010 and 5.30.020 upon payment of a filing fee of $25.00; provided, that only one such fee shall be charged when a declaration of intent and financial statement are filed together. An organization requesting a single function authority shall pay a fee of $15.00 which shall cover licensing and filing fee. (Ord. 860 § 3, 1972).
