16.30.010 Ambulance transport rates.
The following rates are established for ambulance transports performed by the city of Puyallup:
Basic Life Support (“BLS”): base rate of $600.00 plus $12.00 per mile; and
Advanced Life Support, Level 1 (“ALS 1”): base rate of $800.00 plus $12.00 per mile; and
Advanced Life Support, Level 2 (“ALS 2”): base rate of $900.00 plus $12.00 per mile.
(The terms “BLS,” “ALS 1” and “ALS 2” shall be as defined in the Federal Register, Vol. 67, No. 39, February 27, 2002, p. 9133, at Section 414.605, or as may subsequently be amended).
Rates may be amended from time to time by the city council. A copy of current rates shall be maintained by the city clerk. (Ord. 2907 § 1, 2008; Ord. 2871 § 1, 2006; Ord. 2832 § 1, 2005; Ord. 2791 § 1, 2004).
16.30.020 Authority to promulgate regulations.
The city manager is hereby authorized to promulgate any and all regulations necessary to implement the provisions of this chapter, including without limitation requirements for billing, payment, collections, and penalties for delinquent payments. (Ord. 2791 § 2, 2004).
16.30.030 Billing functions and services.
The city manager is authorized to perform billing services necessitated through passage of the ordinance codified in this chapter by using city staff; provided, however, the city manager is hereby given express authority to enter into agreements with other governmental jurisdictions or private companies for the provision of ambulance billing services. 16.30.040 Violation – Penalty.
(1) It is a civil infraction for any person to fail to pay charges assessed in accordance with this chapter or to violate a provision or regulation adopted under authority of this chapter.
(2) Each such person is deemed to have committed a separate offense for each and every day or portion thereof during which a violation of any of the provisions of this chapter is committed, continued, or permitted.
(3) A person convicted of a violation of the provisions of this section is guilty of a Class 1 civil infraction. (Ord. 2791 § 4, 2004).
