APPLICATION FOR SERVICE

13.12.020 Request for disconnection (removal of meter, discontinuation of service).
13.12.010 Application.
A. The city will require prospective customers to sign an application for water service. The application must be made in writing on a standard form at the office of the water department. The application shall set forth:
1. The signature of the applicant;
2. The owner of the premises to be served;
3. The location of the premises;
4. The address of the party paying the bills;
5. The size of the service pipe required;
6. The purpose for which the water is to be used.
B. The application is merely a written request for service and does not in itself bind the city to serve.
C. The city reserves the right to serve water upon any premises regardless of classification through either metered or flat rate service. (Ord. 2016-001 § 1 (Exh. A); Ord. 164 § 2, 1953)
13.12.020 Request for disconnection (removal of meter, discontinuation of service).
A. Any request for disconnection of service shall be accepted only from the owner of the premises and shall set forth:
1. The signature of the applicant certifying under oath (RCW 9A.72.085), that they are the owner of the property being served.
2. A certification that the owner is aware that any future reconnection shall require payment of the difference between general facility charges paid upon the previous connection and the new general facility charges in effect at the time of reconnection (c.f. Chapter 13.72 SMC).
3. The location of the premises with legal description and parcel number;
4. The address of the party paying the bills.
B. The disconnection fee shall be paid.
C. A fee for recording the application and certification with the Clallam County auditor shall be paid. (Ord. 2016-001 § 1 (Exh. A))
